Community Support Services present a range of supports necessary to assist the eligible individual in achieving and maintaining valued life roles in the social, employment, physical health, educational and/or housing domains. Licensed clinical staff partner with individuals to assess their rehabilitative needs and ensure individualized rehabilitative plans are established. The staff offer services and supports necessary to assist the individuals in restoring or maintaining their level of functioning that will allow them to achieve community integration and to remain in an independent living setting of their choice. Individuals are offered opportunities to improve their skills, self-esteem, and sense of belonging to the larger community. These services are provided within a framework that promotes psychiatric rehabilitation and wellness and recovery.
